Microneedling vs Chemical Peel: Key Differences

Microneedling makes tiny punctures with needles. A chemical peel puts an acid solution on the skin. In two randomized studies, microneedling alone did better than a peel alone for acne scars, and in small studies combinations did best. There is no winner for everyone.

Microneedling vs chemical peel, from the sources cited

What studies comparedAcne scars. In 60 patients, microneedling alone did better than a Jessner's solution peel alone, and the two together did best (1). In 60 patients with darker skin tones, 73.33% of those who had microneedling gained more than one grade on a scarring scale, against 33.33% with a 35% glycolic acid peel (2).
RecoveryExpected effects of redness, swelling, pain and dryness lasting up to 7 days (4)Depends on the strength and depth. Ask the professional.
RisksBleeding, bruising, redness, itching, peeling, dark or light spots, infection (5). Active infection, darker skin and metal allergies raise the risk (4).The authors of the glycolic acid study note that current acne scar treatments can cause complications in darker skin, including pigment changes and further scarring (2).
CombinationIn a split-face study of 24 volunteers, a roller combined with a 15% peel did better than the roller alone, and better than the roller combined with PRP (3).

There is no winner for everyone. The studies are small, they tested specific peels, and they looked at acne scars, not every use.

Different mechanisms

Microneedling is mechanical. A peel is chemical. The FDA lists microneedling effects such as bleeding, bruising and peeling (5). A peel can also cause peeling and irritation, in a different way. Which suits you depends on the skin concern, your skin tone, your medicines and your professional's experience.

Questions to ask

  • Which of the two do you suggest for my concern, and why?
  • Would you combine them, and in what order?
  • How should I prepare, and what should I avoid?
  • What does my skin tone change?
  • What does the written quote include?

Common questions

Which is better for acne scars?

In two randomized studies of 60 patients each, microneedling alone did better than the peel alone, but the peels were specific ones and the groups were small (1, 2). In another, combining a roller with a 15% peel did better than the roller alone (3). There is no universal winner.

Can I have both?

Some professionals combine them, and small studies tested combinations. Whether and when to combine depends on your skin and your professional's plan. Ask.

Which has less downtime?

It depends on the depth and the strength used. For microneedling, a 2021 review describes expected effects lasting up to 7 days (4). We did not find a source on peel recovery that we could cite here, so ask the professional about the peel being proposed.
